Scheduling or Rescheduling an Appointment? Here’s What You Need

We hope that you’ll never have to reschedule your appointment, but we understand that sometimes life happens. Whether rescheduling due to illness, work, or other schedule conflicts, the Clearwater Dental team tries to make rescheduling as easy as possible. You can call our office or respond through our text or email confirmations to let us know when you can’t make it.

For new patients calling to set up a first appointment, we ask that you have your insurance information handy – this helps save time when you check in for your appointment. All we need is the insurance provider, your member ID number, as well as which employer it is through (if any). If you are not the policyholder on the insurance, we will need the full name and date of birth of the policy holder in order to add it to the account. If you have a picture of the insurance card, you can email it to: info@clearwaterdentalec.com and we will get it added to your account before your appointment.

For current patients calling to schedule or reschedule an appointment, please have your calendar on hand with your work schedule, school schedule, or a general idea of days or times that are best for you. We get changes to our schedule daily, so ask us about our call list for certain days or times that may be full. The call list is the best way to get in as soon as possible, whether for a cleaning or any other treatment, you may need to schedule with our dental practice in Eau Claire, Wisconsin.
